PLEASE, ARRIVE AT THE AIRPORT two (2) hours prior to your departure time. You must have a government issued ID with your photo (driver's license, State ID, passport, etc.) Give the Record Locator Number or Reservation Number to the person at the ticket counter to obtain your/boarding pass. A staff member well be waiting at Baggage with your name on a sign.If you have any questions call our business office at 1-888-608-0011 between the hours of 8:00am - 5:00pm.

2.     Insurance card (or copy of policy holder’s card)

3.    Prescription Card

4.    Current medications/refill prescriptions

5.    Spending money (for personal items, prescription co-pays, etc.)

6.    Cell phone/Calling card*
      *Cell phones are turned in upon arrival, but can be accessed through your therapist.

7.    Clothing* (approximately 7 days worth)
       *Casual, comfortable but appropriate (e.g. nothing depicting drugs, sex, etc.) attire

• Shorts, t-shirts, jeans, tennis shoes, light jacket or sweater, etc.
• Exercise attire for the gym
• Bathing suit and beach towel

8.    Personal hygiene items
      *All items must be ALCOHOL FREE
